Instructions for typing in form fields and sending

  1. Right click on link below, choose ‘Save Link As’ and save the PDF to your computer.
  2. Open the downloaded file with Acrobat Reader and fill out the form. (A box may pop up that says “Form Can Be Filled and Signed Electronically.” Close this box.)
  3. Print the filled out form.
  4. Mail the printed, signed and dated form to:
